Nootropics in South Africa: Evidence-Based Brain Supplements That Actually Work

"Nootropic" has become one of the most abused terms in the supplement industry. Originally coined in 1972 to describe compounds that enhance cognitive function without significant side effects, the term now gets applied to everything from caffeine tablets to exotic peptides. Here's a ground-up, evidence-based guide to what actually works.


What Makes a True Nootropic?

The original definition (from pharmacologist Corneliu Giurgea, who coined the term) requires that a true nootropic:

Lion's Mane Mushroom (Hericium erinaceus)
The most convincing natural nootropic. Lion's mane stimulates Nerve Growth Factor (NGF) — a protein essential for neuronal growth, maintenance and survival. Clinical trials show significant improvements in cognitive function and memory in adults with mild cognitive impairment. L-Theanine
An amino acid found naturally in green tea, L-theanine promotes alpha brainwave activity — the state associated with calm, alert focus. It doesn't sedate; it smooths. Most meaningfully, L-theanine blunts the anxiety-inducing effects of caffeine while preserving its attention-enhancing benefits. The L-theanine + caffeine combination is one of the most consistently effective and well-tolerated cognitive stacks in the literature. Dose: 100–200mg.

Ginkgo Biloba
One of the most studied herbs for cognitive function. Ginkgo improves cerebral blood flow, has antioxidant properties, and has demonstrated meaningful benefits in multiple clinical trials for memory, processing speed and attention — particularly in older adults. A 2016 Cochrane review found consistent evidence for ginkgo improving symptoms of mild cognitive impairment. Dose: 120–240mg standardised extract (24% flavone glycosides, 6% terpene lactones).

NADH (Nicotinamide Adenine Dinucleotide)
NADH is a coenzyme central to cellular energy production (the last step of the electron transport chain). The brain is the most metabolically demanding organ in the body — accounting for 20% of the body's energy use while comprising only 2% of its mass. NADH supplementation has been studied for cognitive fatigue, jet lag, and early Parkinson's disease. Evidence is promising if not definitive. Useful for those with energy-related cognitive dysfunction rather than healthy young adults.

Shilajit
A mineral-rich resin from the Himalayas, shilajit contains fulvic acid and 80+ trace minerals. Fulvic acid acts as an electron donor/acceptor and may enhance mitochondrial function and NADH activity. A 2012 RCT found shilajit significantly improved cognitive function in healthy volunteers. It's also a potent adaptogen, reducing cognitive decline under stress. Often combined with lion's mane and sea moss.

Methyl B12 (Methylcobalamin)
Vitamin B12 deficiency is extremely common (especially in vegetarians, vegans, and those over 50) and directly causes cognitive decline, brain fog, and peripheral neuropathy. Methylcobalamin is the neurologically active form — it crosses the blood-brain barrier more effectively than cyanocobalamin (the cheap form in most multivitamins). If you have unexplained brain fog, B12 deficiency should be ruled out first.

CoQ10 (Coenzyme Q10)
Like NADH, CoQ10 is central to mitochondrial energy production. Brain cells have exceptionally high mitochondrial density. CoQ10 supplementation has demonstrated benefits for cognitive function in older adults, and is a key supplement for those on statins (which deplete CoQ10). Dose: 100–300mg ubiquinol form.


What Doesn't Belong in the Nootropics Category

Most commercial "nootropic blends" — Frequently contain 15–20 ingredients at doses too low to have any effect, while the marketing suggests synergistic benefits. The evidence for these proprietary stacks is almost always absent.

Stimulant-heavy products — Caffeine, synephrine, and similar compounds improve alertness but are not nootropics — they don't enhance cognition, they suppress fatigue. The benefit disappears with tolerance and rebounds as worse fatigue.

Racetams without medical supervision — In South Africa, most racetams (piracetam, aniracetam, etc.) are unscheduled but also unstudied in the general healthy population. Their evidence base is primarily in patients with cognitive impairment.
