Omega-3 Supplements in South Africa: Fish Oil, Krill Oil and Plant-Based Options

Omega-3 fatty acids are among the most researched nutrients in nutritional science — and among the most deficient in the modern South African diet. The evidence for supplementation spans cardiovascular health, inflammation, brain function and mental health. But not all omega-3 supplements are created equal. Here's what you actually need to know.


What Are Omega-3 Fatty Acids?

Omega-3s are a family of polyunsaturated fats. Three forms are important for human health:

EPA (eicosapentaenoic acid) — Found in fatty fish and marine sources. Primary role: anti-inflammatory. EPA reduces the production of inflammatory eicosanoids and cytokines — directly relevant to cardiovascular risk, joint inflammation, and depression.

DHA (docosahexaenoic acid) — Also found in marine sources. Primary role: structural. DHA is a key structural component of brain cell membranes (making up ~40% of polyunsaturated fat in the brain), the retina, and the heart. Critical during pregnancy and early childhood development.

ALA (alpha-linolenic acid) — Found in plant sources (flaxseed, chia, hemp, walnuts). ALA must be converted to EPA and DHA in the body. The conversion rate is poor: typically only 5–10% of ALA converts to EPA, and less than 1% to DHA. Plant-based omega-3 sources can contribute to ALA intake but should not be relied on as the sole omega-3 source.


What the Research Says

The evidence base for omega-3 supplementation is extensive:

Cardiovascular health
A 2019 large-scale randomised trial (REDUCE-IT, n=8,000+) found that high-dose EPA supplementation reduced major cardiovascular events by 25% in people with elevated triglycerides. Multiple meta-analyses confirm omega-3s reduce triglycerides (by 20–30% at high doses), lower blood pressure, and reduce cardiovascular mortality risk.

Brain health and depression
DHA is a structural component of the brain. Low omega-3 status is consistently associated with higher rates of depression. A 2019 meta-analysis of 26 RCTs found omega-3 supplementation significantly reduced depressive symptoms — with EPA appearing to be the more therapeutically active component.

Joint inflammation
Multiple RCTs show omega-3 supplementation significantly reduces morning stiffness, joint tenderness and NSAID use in rheumatoid arthritis. Anti-inflammatory effects extend to other inflammatory conditions including IBD and psoriasis.

Eye health
DHA makes up ~50% of the fatty acid content of the retinal photoreceptors. Adequate DHA is associated with reduced risk of age-related macular degeneration.

Pregnancy and infant development
DHA is critical during the third trimester and early infancy for brain and retinal development. Adequate maternal DHA intake is associated with better cognitive outcomes in children. The recommended intake during pregnancy is 200mg DHA daily above normal adult requirements.


Fish Oil vs Krill Oil vs Plant-Based: Which Is Best?

Standard fish oil — The most studied form. Look for high EPA+DHA content per capsule (aim for 500–1,000mg combined EPA+DHA per serving, not just total oil). Quality matters: cheap fish oil can be oxidised (rancid), negating benefits and potentially causing harm. Look for third-party testing and IFOS certification.

Krill oil — Omega-3s are bound to phospholipids rather than triglycerides. This may improve absorption (particularly to the brain, which prefers phospholipid-bound DHA). Contains astaxanthin — a potent antioxidant that also protects the oil from oxidation. Smaller capsules, less fishy aftertaste. Higher cost per gram of EPA+DHA.

Cod liver oil — A traditional source combining EPA, DHA with fat-soluble vitamins A and D. Good for those who want vitamin D alongside omega-3. Note that high-dose cod liver oil can deliver excessive vitamin A — a concern at therapeutic doses.

Plant-based (algae oil) — DHA sourced directly from marine algae — where fish get their DHA in the first place. The only plant source that provides preformed DHA (not just ALA for conversion). Suitable for vegetarians and vegans. Provides DHA; EPA content varies by product.


Omega-3 Dosage Guide

  • General health maintenance: 500–1,000mg combined EPA+DHA daily
  • Anti-inflammatory / joint / heart: 2,000–3,000mg EPA+DHA daily
  • Depression support: 1,000–2,000mg EPA daily (EPA is the clinically relevant component for mood)
  • Pregnancy: 200–300mg DHA daily above baseline

Take omega-3 with a meal containing fat to maximise absorption. Enteric-coated capsules reduce fishy reflux.
