The story

Moducare grew out of immunology research conducted by Professor Patrick Bouic and his colleagues at the University of Stellenbosch Medical School in the late 1980s and 1990s. The team established that a precise 100 to 1 ratio of beta-sitosterol to beta-sitosterol glucoside (a sterol and its glucoside) produced a measurable immune-modulating effect: dampening overactive responses while supporting underactive ones. This was not a stimulant. It was a regulator.

The formulation went through years of double-blind placebo-controlled trials, with results published in peer-reviewed immunology and integrative medicine journals. That clinical paper trail is unusual in the natural health category and is the reason Moducare became one of the most prescribed sterol products globally. Today it is part of Aspen Pharmacare's South African portfolio and continues to be the reference plant sterol formulation for immune balance, including in people with allergies, autoimmune tendencies, chronic stress patterns, and post-infection recovery.
We were not looking for a stimulant. We were looking for a molecule that could help the immune system regulate itself, dial up the underactive arm and dial down the overactive arm. The 100 to 1 sterol-sterolin ratio is what produced that effect in the lab and in the trials.

What makes them different

  • 01

    Clinically Trialled

Subject of double-blind placebo-controlled trials with results in peer-reviewed journals. One of the few South African natural health products with that level of published clinical evidence.

  • 02

    Immune Modulator, Not Stimulator

The 100:1 sterol to sterolin ratio targets immune balance. It does not push an already overactive immune system harder, which makes it relevant for allergies and autoimmune-leaning conditions.

  • 03

    Stellenbosch Research Origin

Developed inside Professor Patrick Bouic's immunology lab at the University of Stellenbosch Medical School. Decades of South African scientific work behind the formulation.

  • 04

    Pharmacy Backed

Now part of Aspen Pharmacare's portfolio. Manufactured to pharmaceutical-grade standards and distributed through pharmacies and clinical channels nationally.

Pair with allergy or stress support

Moducare is often layered with other targeted support depending on the use case: antihistamines or quercetin for allergies, adaptogens or magnesium for stress-related immune fatigue. The sterols handle the balance question; the layered support handles symptoms.

Practitioner-guided protocols

For autoimmune patterns, chronic infection recovery, or stubborn allergy seasons, Moducare is most commonly used as part of a structured 3 to 6 month protocol under a doctor or integrative practitioner. The clinical trial doses were sustained, not occasional.

About this brand

Common questions customers ask about the brand and its products.

What does 'immune modulator' actually mean?
A modulator regulates the immune response in both directions: it supports underactive immunity (poor infection response, slow recovery) and dampens overactive immunity (allergies, autoimmune tendencies). This is different from an immune booster, which only pushes immune activity higher and is not appropriate for everyone. Moducare's sterol-sterolin combination targets the regulatory arm specifically.
How long until I notice an effect?
The clinical trials ran on sustained dosing over weeks to months. Most users do not notice anything dramatic in the first week. By weeks four to eight, the effect is typically subtle but consistent: fewer allergy flare-ups, fewer minor infections, better recovery from stress. It is a baseline-shifting supplement, not an acute one.
Why does it need to be taken on an empty stomach?
Plant sterols are absorbed in the same intestinal pathway as dietary fats, including the fat in food. Taking Moducare with food causes the sterols to compete with dietary fats for absorption and dramatically reduces uptake. Empty stomach (one hour before or two hours after eating) ensures the sterols are absorbed in the intended quantity.
Is Moducare safe with chronic medication?
Moducare has been used alongside conventional medication in clinical settings for decades with no major documented interactions. If you are on immunosuppressive medication for autoimmune disease or post-transplant, speak to your specialist before starting, because modulating the immune system is the point and the interaction needs to be supervised.
Can children take Moducare?
Moducare has been used in paediatric allergy and immune support contexts, but dosing should be guided by a paediatrician or integrative practitioner. The adult dosing on the label is not appropriate for small children.
Who is Professor Patrick Bouic?
Professor Patrick Bouic is a South African immunologist who led the research programme at the University of Stellenbosch Medical School that established the immune-modulating effect of the 100:1 sterol-sterolin ratio. His published work in the 1990s and 2000s is the clinical foundation of the Moducare formulation.
How does Moducare compare to high-dose vitamin C or echinacea?
Vitamin C and echinacea are short-term immune stimulants typically used at the onset of a cold or flu. Moducare is a long-term immune regulator taken daily for weeks to months. They are different tools for different situations and are sometimes used together, with Moducare as the baseline and acute stimulants layered in when needed.
